Request an RMA for Product Repair or Verification Test

If you would like to return equipment for repair or verification testing, please follow the following steps to initiate the request:

  1. Click the link to the interactive PDF form linked below
  2. Download to your computer – do not fill out in the browser and then download as it will not save the data before downloading
  3. Open in Adobe Acrobat Reader or similar application (not an internet browser)
  4. Fill out the PDF per its instructions
  5. Click the button on Page 2 of the form to open in your mail client and send back to Accumetrics. If the button doesn't work, please send back to telemetry@pcb.com.
